Request Form - Non-SPS Staff Laptop

Please do not submit a request here for SPS employees. Those requests will be closed.

Only administrators of the school building should submit this request which will then be reviewed by the OITA office. Once reviewed, an email will be sent back to the user making the request to let them know if the request was approved or not.

  • If approved, the person who submitted the Service Request will receive an email telling them to schedule a laptop appointment or to have the Head Clerk do so.
  • If denied, we will ask you to resubmit your request with the necessary corrections.
