Request Form - Creation of Microsoft Team

*Attention*

Only SPS staff should submit this request, which the OITA department will review for approval. Once reviewed, an email will be sent back to the user who submitted the request to let them know if it was approved.

*Please submit only one form per each Team needed*

• If approved, you will receive an email stating your request has been approved and your Team has been created. The Team will be accessible only to the user who was designated as the Team owner on the request form.
• If denied, you will receive an email stating your request is denied. If you are denied and would like to know why, please open a TeamDynamix ticket for assistance: Service - Microsoft 365 Support (teamdynamix.com).
