Request Form - Creation of Microsoft Team

*Attention*

Only SPS staff members should submit this request, which will be reviewed by the OITA department for approval. Once reviewed, an email will be sent back to the user who submitted the request to inform them of the approval status.

*Be advised that approval can take up to one to three business days.*

  • If your request is approved, you will receive an email stating your request has been approved and your Team has been created. The Team will be accessible only to the user who was designated as the Team owner on the request form.
  • If your request is denied, you will receive an email stating that your request is denied. If you are denied and would like to know why, please open a TeamDynamix ticket for assistance: Service - Microsoft 365 Support (teamdynamix.com).
