Request Form - DHCP Reservation / Static IP

*Attention*

 

This request form requests a DHCP Reservation / Static IP from the SPS Technology Operations department.

  • Requests are only authorized for core building infrastructure (e.g., HVAC, Alarm Panels, Lighting, etc.).
  • Static IP addresses are provided when the device cannot utilize a DHCP reservation. 
  • Requests may take up to 10 business days after the network cable run has been completed.
  • IP assignments will not be provided until the network cable is run, terminated, labeled, and patched.
  • Incomplete information may result in a delay or rejection of the request.
